COVID-19: key developments for Monday, March 16, 2020

A collection of key developments in the fight against COVID-19 (the actual virus is SARS-COV-2), posted throughout the week for those who just want the signal and not the noise. If there’s something you think we should include, sound off in the comments thread attached to the post.

Today was the day the city-wide lockdowns started in earnest in the US.

San Francisco and multiple nearby counties announced total lockdowns, so that people stay home unless they absolutely have to. More here in the SF Chronicle.

New York, New Jersey, and Connecticut are shutting all bars, restaurants, and movie theaters, and there are rumors of a quarantine NYC where no one is allowed in or out (but those remain rumors, for now).

Los Angeles and Washington, DC, have closed public buildings.

There are rumors swirling of a domestic flight shutdown, but this would be pretty dramatic since it would shut down so much mail order commerce. (Passenger planes carry cargo, as well.) If we go there, I don’t think it’ll be for a while, yet.

Continuity of government is always a concern in a crisis like this. Trump and Pence are sticking with each other, but the Pentagon is not taking any chances.

NASCAR is postponing all races through at least May 3rd.

A new paper in Science claims that asymptomatic infection is widespread and is in fact driving the epidemic. Substantial undocumented infection facilitates the rapid dissemination of novel coronavirus (SARS-CoV2).

We estimate 86% of all infections were undocumented (95% CI: [82%–90%]) prior to 23 January 2020 travel restrictions. Per person, the transmission rate of undocumented infections was 55% of documented infections ([46%–62%]), yet, due to their greater numbers, undocumented infections were the infection source for 79% of documented cases. These findings explain the rapid geographic spread of SARS-CoV2 and indicate containment of this virus will be particularly challenging.

      Guys, this source of “information” is: Unnamed closed Facebook groups (allegedly for physicians) -> anonymous physician -> Mother Jones magazine.  The credibility is close to zero with this chain.   Please pass along useful and actionable intel, not fear mongering.

        There is no chain of information, what are you talking about? It’s an interview with someone who the magazine gave anonymity. It’s not as though they’re pretending like they’re giving some secret information; it’s just general reflections on how doctors are feeling. If you’re going to take some bizarre stance wherein giving a source anonymity = zero credibility, fine, but don’t pretend like that’s a normal position!

        I’ve been in touch privately with a number of healthcare workers, and with other journalists who’ve been in touch with healthcare workers, and we’re all hearing very similar things to what’s in this article. Nothing in the piece seems off to me or doesn’t fit with my own reporting, and I think it’s good and worth sharing.

        Healthcare workers are good people and like all other human beings, just as prone as anyone else to spread gossip, rumors and FUD.  Good data with specific and verifiable information is far more preferable than the chain of Facebook group -> anonymous physician.

        FUD in this situation is even worse than having no information.  It will push people to make incorrect decisions.

        I hear both sides, though the MJ piece read (to me) as slanted towards “COVIDOMG!!” rather than an objective rendering. On the one hand, I hear that HCW folks might feel reticent speaking critically on the record in a time of crisis. On the other hand, this isn’t China, they needn’t fear retribution (especially at this stage) and let’s be honest: nobody is firing any HCW anytime soon no matter what they say. Jon, I get what you mean by the story validating your own anecdotal experience, but I did read it and my first thought was, “hmmm, not super congruent with what I’ve come to expect from The Prepared”. n =1, however, and, as always, incredibly grateful for this resource.

      The article in Science is more interesting, but reading it reveals that the estimate that 86% of all infections were undocumented is based upon a math model of time and geography.  I do big data and math models professionally and these models usually work only in narrow and well understood spaces.  I find it dubious that COVID-19 is such an animal.
